Dental Creations, Inc. (“Dental Creations,” “we,” “our” or “us”) is a dental laboratory in Revere, Massachusetts. This policy explains what information we collect through dentalcreationsinc.com (the “site”), how we use and store it, who we share it with and the choices you have. The site is intended for licensed dental professionals and their staff.

Information we collect

Information you give us

Contact form. Your name, email address, and optionally your phone number, practice name, the reason for your inquiry, the products you are interested in and your message.
Online case submission. The doctor’s name, practice name and address, email, phone and license number; the patient’s initials or your case ID; prescription details such as teeth, restoration type, materials, shade and implant information; your instructions; and any files you upload, such as intraoral scans, photographs and prescription documents.
Email and phone. Anything you send us directly, including scans and prescriptions sent by email.

Information collected automatically

Server logs. Like most websites, our hosting provider records technical information for each request, such as IP address, browser type, the page requested and the time. These logs are used for security, troubleshooting and preventing abuse.
Submission records. When you send the contact form or submit a case, we record the date and time and the IP address it came from with that message or case record.
Spam and abuse protection. To limit repeated or automated submissions, we temporarily store a one-way hashed value derived from your IP address and the times of recent submissions. These entries expire within an hour.

Cookies and tracking

The site does not set cookies and does not use analytics, advertising or social media tracking tools. Fonts and scripts are served from our own site rather than third-party services. If we add analytics in the future, we will update this policy first.

Patient information

Case submissions may include information about your patients. We ask that you share only what is needed to fabricate the restoration: use the patient’s initials or your own case ID instead of a full name where possible, and avoid including unrelated health history. We use patient-related information only to design, fabricate, deliver and support the restoration you prescribed, and to keep the business records related to that case. Please do not include patient information in the general contact form.

To schedule pickups, deliveries and shade appointments
To send case confirmations and other messages about your cases
To invoice and keep business records
To protect the site, our systems and our clients against fraud, spam and abuse

We do not sell personal information, and we do not use it for advertising.

How we store and protect it

The site is served only over encrypted HTTPS connections, so forms and uploads are encrypted in transit.
Uploaded case files are stored on our hosting account in a private location that is not publicly accessible from the web.
The lab is notified of new cases by email. That email includes the prescription details and download links for the uploaded files. Each link is unique, works only for that file and expires after 180 days.
Access to case files and records is limited to Dental Creations staff and the service providers listed below.

No method of transmission or storage is completely secure, but we take reasonable administrative and technical measures to protect the information you send us.

How long we keep it

We keep case submissions, uploaded files and related correspondence for as long as needed to fabricate, deliver and support the restoration, including remakes and adjustments, and to meet our business, tax and legal record-keeping obligations. Contact form messages are kept as part of our ordinary correspondence. When information is no longer needed, we delete it.

Who we share it with

We share information only with service providers that help us run the site and the lab, and only as needed for them to provide their services:

Website hosting: Hostinger, which hosts the site, stores uploaded files and processes server logs.
Email delivery: our email providers, which deliver contact form messages, case notifications and confirmations.
Shipping carriers: for practices outside our local delivery area, the information needed to ship a case.

We may also disclose information when required by law, to respond to legal process, or to protect the rights, property or safety of Dental Creations, our clients or others.

Your choices

You can ask us to access, correct or delete personal information you have given us by contacting us using the details below. We will respond within a reasonable time. We may need to keep certain records, such as invoices or case records, where the law requires it or where they are needed to support work we have delivered.

Children

The site is for dental professionals and is not directed to children. We do not knowingly collect personal information from children under 13 through the site.

Changes to this policy

We may update this policy as the site or our practices change. The effective date at the top of this page shows when it was last revised.
